Food Sensitivity Test
One (1) Initial Functional Health Consultation
One (1) Laboratory Results Review Appointment
Four (4) Follow-Up Sessions

All services must be completed within 12 months of enrollment unless otherwise agreed upon in writing.

I agree to maintain an active relationship with my Primary Care Provider and understand this package is complementary to, not a substitute for, conventional medical care.

I understand this package does not include medical diagnosis or pharmaceutical prescribing and is intended for educational and wellness purposes only.
