WHAT IS INCLUDED
The Functional Gut Health Package includes:
• One comprehensive initial functional-health consultation.

• One dedicated appointment to review both tests and develop a personalized treatment plan.
• Three additional functional-health follow-up appointments.
• Five appointments total.
• Reasonable secure-message support for brief questions related to the active treatment plan. Messaging does not replace an appointment
The GI-MAP evaluates the gastrointestinal environment and may provide information about H. pylori, beneficial and opportunistic bacteria, parasites, yeast or fungal markers, digestion, intestinal immune function, inflammation, and intestinal permeability.
The IgG food-sensitivity test evaluates immune reactivity to selected foods. It is not an allergy test and does not diagnose an immediate or IgE-mediated food allergy.
Recommendations may include nutrition, temporary food elimination and reintroduction, lifestyle changes, digestive support, supplements, herbs, sleep support, stress management, and referrals when appropriate.
DISCOUNTED ACUPUNCTURE
Patients enrolled in this program may receive up to twelve (12) acupuncture follow-up visits for $85 per visit instead of the regular $100 follow-up price.
Discounted visits:
• Must be used within one year of the original program purchase date.
• Apply only to the patient enrolled in the program.
• Cannot be shared, transferred, assigned, sold, or gifted.
• Have no cash value.
• Do not include initial acupuncture evaluations, injections, supplements, herbs, or specialty services.
Regular cancellation and no-show policies apply.
WHAT IS NOT INCLUDED
The package price does not include:
• Supplements, herbs, food, medications, hormones, acupuncture, massage, injections, imaging, or outside professional services unless specifically stated.
• Prescription or pharmaceutical management.
• Additional, repeat, upgraded, or follow-up testing.
• Blood-draw fees, replacement kits, recollection costs, expedited shipping, or costs resulting from collection or shipping errors.
• Unlimited appointments.
• Emergency, primary-care, specialist, hospital, or psychiatric services.
PATIENT RESPONSIBILITIES
The patient agrees to:
• Provide complete and accurate health, medication, supplement, allergy, pregnancy, and medical-history information.
• Review all testing instructions and complete each test correctly and within the recommended timeframe.
• Contact IWC before collecting if the kit is damaged, instructions are unclear, medications or hormones have changed, pregnancy is possible, or the required preparation cannot be followed.
• Properly label, store, package, and ship specimens.
• Retain shipment tracking information.
• Pay replacement, recollection, laboratory, or shipping costs when a specimen is unusable because instructions were not followed.
• Attend the test-review and follow-up appointments.
• Make a reasonable effort to follow the treatment plan and communicate questions, side effects, or barriers.
• Maintain appropriate care with a primary-care physician and other medical specialists.
Raw laboratory reports may be available before the review appointment. Patients should not begin, stop, or change treatment based solely on a raw report without clinical review.
APPOINTMENTS AND PROGRAM USE
Appointments are subject to IWC’s current cancellation, late-arrival, and no-show policies. A missed or late-cancelled appointment may be forfeited or require an additional fee and does not create a right to a refund.
All included services must be completed within the program period established by IWC. Unused appointments, testing, or services have no cash value.
PAYMENT AND REFUND POLICY
The program must be paid in full at enrollment or through an approved third-party financing option.
A written refund request must be received within twenty-four (24) hours of purchase and before any test has been ordered, laboratory requisition submitted, kit registered, appointment completed, or other nonrecoverable expense incurred, whichever occurs first.
For an approved refund, the patient is responsible for the actual credit-card, payment-processing, and refund-processing charges assessed to IWC. These charges will be deducted from the refund.
After the 24-hour period or once ordering or services have begun, all sales are final and no refunds will be issued.
No refunds or credits will be provided for:
• Failure to complete testing.
• Missed testing
• Missed appointments.
• Failure to follow the treatment plan.
• Change of mind, relocation, or choosing another provider.
• Improvement, lack of improvement, worsening symptoms, or dissatisfaction with recommendations.
This package is for one patient only. It may not be divided, shared, transferred, assigned, sold, or gifted. Unused components are not separately refundable.
NO GUARANTEE AND SCOPE OF CARE
No diagnosis, cure, symptom resolution, fertility result, weight change, laboratory improvement, or specific timeline is guaranteed.
IWC does not prescribe pharmaceuticals or manage prescription medications through this program. Patients should never discontinue or change a prescribed medication unless directed by the prescribing clinician.
This program complements but does not replace medical care from a primary-care physician or specialist.
Call 911 or seek urgent medical care for severe or rapidly worsening symptoms. IWC messaging is not an emergency service.
ELECTRONIC ACKNOWLEDGMENT
By electronically signing or selecting the acknowledgment in IWC’s electronic health record, the patient confirms that they have read and understand this Agreement; understand what is and is not included; accept the testing, appointment, participation, payment, and refund requirements; understand that the package and acupuncture discount are for one patient only and are nontransferable; understand that supplements are not included and pharmaceuticals are not prescribed; understand that results are not guaranteed; and voluntarily consent to participation and to the ordering, receipt, and review of the included tests.
