WHAT IS INCLUDED
The Functional Hormone Health Package includes:
• One comprehensive initial functional-health consultation.
• DUTCH Hormone Test.
• GI-MAP comprehensive stool test.
• One dedicated appointment to review both tests and develop a personalized treatment plan.
• Three additional functional-health follow-up appointments.
• Five appointments total.
• Reasonable secure-message support for brief questions related to the active treatment plan. Messaging does not replace an appointment and is not available for emergencies.
The DUTCH test may evaluate reproductive hormones, hormone metabolites, androgen metabolism, DHEA, free and metabolized cortisol, cortisol patterns, cortisol clearance, and selected nutritional or oxidative-stress markers.
Testing must be performed at the appropriate time based on menstrual-cycle status, ovulation, pregnancy, hormone use, and the specific panel ordered.
The GI-MAP may provide information about H. pylori, beneficial and opportunistic bacteria, parasites, yeast or fungal markers, digestion, intestinal immune function, inflammation, and intestinal permeability.
Recommendations may include nutrition, blood-sugar support, digestive support, hormone-metabolism support, menstrual or perimenopausal support, supplements, herbs, sleep support, stress management, and referrals when appropriate.
DISCOUNTED ACUPUNCTURE IF DESIRED
Patients enrolled in this package may receive up to twelve (12) acupuncture follow-up visits for $85 per visit instead of the regular $100 follow-up price.
Discounted visits must be used within one year of the original program purchase date and apply only to the enrolled patient. They cannot be shared, transferred, assigned, sold, or gifted and have no cash value.
The discount does not include initial acupuncture evaluations, injections, supplements, herbs, or specialty services. Regular cancellation and no-show policies apply.
WHAT IS NOT INCLUDED
The package price does not include:
• Supplements, herbs, medications, hormones, acupuncture, massage, injections, imaging, food, or outside services unless specifically stated.
• Pharmaceutical prescribing, hormone prescribing, or medication management.
• Additional, repeat, upgraded, or follow-up testing.
• Blood-draw fees, replacement kits, recollection costs, expedited shipping, or costs resulting from collection or shipping errors.
• Unlimited appointments or messaging .
• Emergency, primary-care, specialist, hospital, or psychiatric services.
• Guaranteed insurance or HSA/FSA reimbursement.
PATIENT RESPONSIBILITIES
The patient agrees to:
• Provide complete and accurate health, menstrual-cycle, hormone, medication, supplement, pregnancy, and medical-history information.
• Complete testing at the correct time and follow all collection instructions.
• Contact IWC before collecting if the kit is damaged, instructions are unclear, ovulation or cycle timing is uncertain, medications or hormones have changed, pregnancy is possible, or preparation cannot be followed.
• Never discontinue a prescribed medication or hormone unless instructed by the prescribing clinician.
• Properly label, store, package, and ship specimens.
• Pay replacement, recollection, laboratory, or shipping costs when a specimen is unusable because instructions were not followed.
• Attend all test-review and follow-up appointments.
• Make a reasonable effort to follow the treatment plan and communicate questions, side effects, or barriers.
• Maintain appropriate care with physicians and specialists.
Raw reports may be available before the review appointment. Patients should not begin, stop, or change treatment based solely on a raw report without clinical review.
APPOINTMENTS AND PROGRAM USE
Appointments are subject to IWC’s current cancellation, late-arrival, and no-show policies. A missed or late-cancelled appointment may be forfeited or require an additional fee and does not create a right to a refund.
All included services must be completed within the program period established by IWC. Unused services have no cash value.
PAYMENT AND REFUND POLICY
The program must be paid in full at enrollment or through an approved third-party financing option.
A written refund request must be received within twenty-four (24) hours of purchase and before any test has been ordered, requisition submitted, kit registered, appointment completed, or other nonrecoverable expense incurred, whichever occurs first.
For an approved refund, actual credit-card, payment-processing, and refund-processing charges assessed to IWC will be deducted where permitted by law.
After the 24-hour period or once ordering or services have begun, all sales are final.
No refund or credit will be provided for failure to complete testing, missed test timing, missed appointments, failure to follow the plan, relocation, change of mind, choosing another provider, improvement, lack of improvement, worsening symptoms, or dissatisfaction with recommendations.
This package is for one patient only and may not be divided, shared, transferred, assigned, sold, or gifted. Unused components are not separately refundable.
NO GUARANTEE AND SCOPE OF CARE
No diagnosis, cure, symptom resolution, fertility result, menstrual-cycle change, weight change, laboratory improvement, or specific timeline is guaranteed.
IWC does not prescribe pharmaceutical medications or hormones through this program. Medication and hormone changes must be discussed with the prescribing clinician.
This program complements but does not replace medical care from a primary-care physician, gynecologist, endocrinologist, obstetric provider, or other specialist.
Call 911 or seek urgent medical care for severe or rapidly worsening symptoms. IWC messaging is not an emergency service.
ELECTRONIC ACKNOWLEDGMENT
By electronically signing or selecting the acknowledgment in IWC’s electronic health record, the patient confirms that they have read and understand this Agreement; understand what is and is not included; accept the testing, appointment, participation, payment, and refund requirements; understand that the package and acupuncture discount are for one patient only and are nontransferable; understand that supplements are not included and pharmaceuticals and hormones are not prescribed; understand that results are not guaranteed; and voluntarily consent to participation and to the ordering, receipt, and review of the included tests.
