Testing options may include GI-MAP, DUTCH, IgG food-reactivity with histamine-clearance testing, mycotoxin testing, environmental toxin testing, Organic Acids Testing, conventional or specialty bloodwork, or endometriosis-related specialty testing.
The provider selects the three clinically appropriate tests. The package does not provide an unrestricted choice of any three tests or laboratory panels. More advanced, upgraded, or higher-cost testing may require an additional payment, which will be discussed before ordering.
DISCOUNTED ACUPUNCTURE
The patient may receive up to 12 acupuncture follow-up visits for $85 per visit instead of the regular $100 price. Visits must be used within one year of the program purchase date. This discount is for the enrolled patient only, is nontransferable, cannot be shared or gifted, and has no cash value.
NOT INCLUDED
The program does not include supplements, herbs, medications, acupuncture, massage, injections, imaging, additional testing, repeat testing, upgraded panels, replacement kits, specimen recollection, blood-draw fees, shipping charges, or outside medical services unless specifically stated.
PATIENT RESPONSIBILITIES
The patient agrees to provide complete health and testing records, follow all testing instructions, complete all three tests within the recommended timeframe, attend scheduled appointments, communicate health or medication changes, and make a reasonable effort to follow the phased treatment plan.
The patient is responsible for replacement or recollection costs if a specimen cannot be processed because the instructions were not followed.
PAYMENT AND REFUNDS
A written refund request must be received within 24 hours of purchase and before any test is ordered, requisition submitted, kit registered, appointment completed, or other nonrecoverable expense incurred, whichever occurs first.
Approved refunds will be reduced by the actual credit-card, payment-processing, and refund-processing fees charged to IWC, where permitted by law.
After 24 hours or once ordering or services begin, all sales are final. No refunds are provided for unused services, missed appointments, incomplete testing, missed collection timing, change of mind, relocation, failure to follow the plan, improvement, lack of improvement, or dissatisfaction with recommendations.
This program is for one patient only and cannot be divided, shared, transferred, sold, or gifted.
MEDICAL DISCLAIMER
Functional and specialty testing has limitations, and results or symptom improvement are not guaranteed. Complex health concerns may require additional medical evaluation or treatment outside IWC’s scope.
IWC does not prescribe pharmaceuticals or replace the patient’s primary-care physician or specialists. Patients should not change prescribed medications without speaking with the prescribing provider.
IWC messaging is not an emergency service. Call 911 or seek urgent care for severe or rapidly worsening symptoms.
ACKNOWLEDGMENT
By electronically signing, the patient confirms that they have read and understand this Agreement; understand that the provider selects the three included tests; understand that advanced testing may cost extra; accept the payment and refund policy; understand that the program is nontransferable; agree to complete testing and appointments; understand that results are not guaranteed; and consent to the ordering and review of the selected tests.
