Monthly Membership Agreement

This Wellness Membership credit includes:

1 Injection (a regular B12 injection or a lipo vite injection) a month
1 Acupuncture treatment monthly.
10% off additional services
20% off professional grade supplements

The conditions for this credit are as follows:

This credit is non-transferable.
The cost is $89 per month and will have a minimum of an 8-month commitment. After eight months, it will continue to be billed unless canceled by emailing us at info@iwcjacksonville.com.
**Note if you are a new patient to the practice, your first visit will have a $46 charge to cover the new patient consultation.

By signing this agreement, you (“Member”) agree to the following terms and conditions: You authorize a charge of $89 on your credit card each month. You understand that the membership is ongoing (min of 8 months) until terminated by either party with 30 days prior written notice. You acknowledge that fees are paid in advance and are non-refundable. You understand that memberships are personal and nontransferrable. You understand that failure to comply with any term or condition of this agreement may result in termination of your membership without refund of fees. By signing you hereby certify that you have read, understood and accepted these terms and conditions.
